The Role of Hyaluronic Acid for Soft Tissue Indications: A Systematic Review and Meta-Analysis
PMID: 35114853
Of the 6930 articles screened, 19 RCTs (n = 1629 patients) were eligible and included in this review. HA was evaluated across a variety of soft tissue indications including rotator cuff disease, elbow pain, ankle sprains, Achilles tendinopathy, patellar tendinopathy, and trigger finger. Of the 19 RCTs, 11 were placebo-controlled and 9 used active comparators (PRP, cortisone, prolotherapy, or extracorporeal shockwave therapy). The pooled treatment effect of HA across most soft indications against placebo and active comparators demonstrated benefit in short-term pain <6 weeks (MD visual analogue scale [VAS] 2.48, 95% CI 2.31-2.65) and 6 to 12 weeks (MD VAS 2.03, 95% CI 1.86-2.20). Mid-term pain relief also favored HA over comparators across indications >12 weeks from administration (MD VAS 3.57, 95% CI 3.35-3.78). High heterogeneity was present with rotator cuff (10 trials, I2 = 94%), and elbow tendinopathy (2 trials, I2 = 99%). We identified uncertain benefit for trigger finger (2 trials, I2 = 67%). Heterogeneity for ankle sprains, patellar tendinopathy and Achilles tendinopathy could not be assessed as they only had 1 trial each.
Conclusion
This systematic review and meta-analysis support HA’s efficacy in the treatment of a variety of soft tissue indications.
Efficacy of platelet-rich plasma injections for symptomatic tendinopathy: systematic review and meta-analysis of randomised injection-controlled trials
doi:10.1136/bmjsem-2017-000237
A total of 16 randomised controlled trials (18 groups) of PRP versus control were included. Median sample size was 35 patients, a study size that would require an effect size ≥1.0 to achieve statistical significance. PRP was more efficacious than control in reducing tendinopathy pain, with an effect size of 0.47 (95% CI 0.22 to 0.72, p<0.001), signifying a moderate treatment effect. Heterogeneity among studies was moderate (I2=67%, p<0.001). In subgroup analysis and meta-regression, studies with a higher proportion of female patients were associated with greater treatment benefits with PRP.
Conclusions Injection of PRP is more efficacious than control injections in patients with symptomatic tendinopathy.

How does Profhilo works? As a stabilized product Profhilo lasts in the skin for around 28 days. During this time the stimulation of 4 different types of collagen and elastin takes place by slow release of HA. The stimulation results in significant tissue improvement.
The high strength hyaluronic acid is injected across strategic points where it will gradually disperse. Profhilo stimulates collagen and elastin, attracting water within the deeper layers of the skin. This provides structural support and hydration.
Profhilo is a fairly new product that uses patented technologies engineered for skin remodelling. IBSA make it in Italy from hyaluronic acid, which is found in many Dermal Fillers. However, rather than filling lines, Profhilo promotes collagen and elastin growth to reduce skin laxity.
Thanks to the unique characteristics of Profhilo, improving skin laxity is easily obtained in only two sessions, with a 1-month interval, and only five injection points on each side of the face. Profhilo is suitable for practically anyone, men and women, from 30 years old and with no upper age limit.
After treatment, Profhilo remains in the skin for around 28 days and slowly releases HA into the skin. This action stimulates the production of 4 different types of collagen and elastin.
The simulation results in significant tissue improvement reducing sagging tissue and signs of ageing, while the hyaluronic acid hydrates the skin.
IBSA recommends 2 sessions with a 4 - 6 week interval by means of the BAP (Bio Aesthetic Points) Techniques to minimise the risks and maximise the product's flowability.
Profhilo delivers great results as a treatment on its own or in combination with other aesthetic treatments. I recommend you avoid other injectable treatments 2 weeks before and 2 weeks after the treatment.
You can have Botulinum Toxin treatment at the same time; otherwise, we recommend a gap of two weeks prior to or 2 weeks after a Profhilo treatment.
Microdermabrasion, chemical peel and IPL can be performed 2 weeks pre or post-treatment. For Fractional resurfacing, a 4 weeks gap between treatments is recommended.
Profhilo treatment is not recommended immediately following other routine medical procedures. For dental procedures, leave at least 2 weeks pre or post-treatment.
Profhilo is a type of injectable treatment used for skin rejuvenation and hydration. It contains a high concentration of hyaluronic acid, a naturally occurring substance in the body that plays a vital role in maintaining skin hydration and elasticity.
At a cellular level, Profhilo works in two main ways:
1. Hydration: Hyaluronic acid in Profhilo attracts and retains water molecules, helping to hydrate the skin from within. As we age, our skin's natural hyaluronic acid levels decrease, leading to dryness and loss of firmness. By injecting Profhilo into the skin, it replenishes the hyaluronic acid content, improving skin hydration and moisture retention.
2. Bio-remodelling: Profhilo is unique because of its specific formulation, which involves a hybrid complex of low molecular weight and high molecular weight hyaluronic acid. This combination is designed to stimulate the production of collagen and elastin in the skin. The low molecular weight hyaluronic acid penetrates deeper into the skin, while the high molecular weight hyaluronic acid acts as a supportive scaffold, creating a bio-remodelling effect. This process encourages the production of new collagen and elastin fibres, leading to improved skin elasticity and firmness.
Overall, Profhilo's dual-action approach of providing intense hydration and stimulating collagen production makes it an effective treatment for addressing skin laxity, fine lines, and overall skin rejuvenation. As with any medical procedure, it's essential to consult a qualified healthcare professional to determine if Profhilo is suitable for your specific skin concerns and needs.
